Support Coordination – coordination of supports will assist you with building your skills needed to understand, use and implement your NDIS Plan.
As part of your support coordination, one of our a dedicated team members will work with you to make sure a variety of supports is used to help you live more independently, maintain relationships, manage daily service delivery tasks and be included in your community.
Support Coordinators who are experienced social workers will help you to get the best out of your plan. They help you to negotiate better rates with your chosen support providers and what service providers would offer you. Support Coordinator will make sure service agreements and service bookings are completed. They will help to build your ability coordinate your supports, access local community and prepare for your plan review.
Specialist Support Coordination - support provided to people who need specialist support and whose needs are more complex.
Specialist Support Coordinator work with you to help you overcome challenges in your daily life and ensure consistent delivery or service. Specialist Support Coordinator is a qualified and experienced practitioner such as Occupational Therapist, Psychologist or Social Worker.
Specialist Support Coordinators support you in managing challenges in your support environment which may include health, education, or justice services. It is their aim to reduce barriers to implementing or using your NDIS plan.
Transition Support Coordination
We understand the importance of life-stage transitions such as leaving school and/or securing a form of employment.
We offer tailored support to assist you with the transition. Without knowing where to turn to, this time can be complex for a young person and their carers to manage. We can assist in planning for this in advance or get involved to resolve points of crisis.
